Immigration Medical Exam
Dr. Arash Lalezary designated by the USCIS as a Civil Surgeon.
During the visit, we will perform the physical examination and assessments, draw blood and urine as needed for testing , then review your vaccination records and update them as required. After your test results are ready, we will complete your I-693 form and have it ready for pick up.

Expedited options available and forms will be ready in 2-3 days +$50 each
USCIS Required Tests (included)
These tests are required by the USCIS and are included in your visit fee
Syphilis (RPR) – required for everyone 15 years and older
Gonorrhea (urine) – required for everyone 15 years and older
Quantiferon Tuberculosis test – required for everyone 2 years and older

Required Vaccinations
Vaccine prices start from $50
USCIS requires all vaccinations to be completed according to the CDC vaccine schedule. This is determined based on each person’s age and the Doctor will review your vaccine records and inform you of any vaccinations that you may need. We have all necessary vaccinations available.
If you were vaccinated but do not have the records, we can run a blood test to check your vaccination status. There is an additional fee for this blood test and the price depends on which blood test you would need.
Children: All childhood vaccination are required to be up to date.
Adults: Depending on the age of the patient, for more details refer to the list below.
USCIS Required Vaccinations
- Hepatitis B – 0 through 59 years of age
- DTaP – 8 weeks through 7 years of age
- Polio IPV – 8 weeks through 17 years of age
- Hib – 8 weeks through 6 years of age
- Pneumococcal – 8 weeks through 6 years of age & 50+
- Hepatitis A – 12 through 18 years old
- MMR – 12 months through 49 years of age
- Varicella – 12 months of age and up
- Tdap – 18 years of age and up
Influenza vaccine – 6 months of age and up
COVID-19 Vaccine – 6 months of age and up
